Annual Complaints review – complete by 31.7.18

Reminder to those practices that still need to complete an annual complaint review.  This is a contractual requirement asking for details of any complaints received between 1st April 2017 to 31st March 2018.  To assist with your return please find a form attached which needs to be completed and returned to us by 31st July 2018 please. This can either be emailed or posted – details are on the form.  Even if you have received no complaints a reporting form needs to be completed please.
